Corneal opacity made cataract surgery difficult,because the illumination by standard surgical microscope could cause scattering and light reflection, and anterior capsule visibility was not enough to perform continuous curvilinear capsulorhexis (CCC). This retrospective observational case series enrolled 11 eyes of 10 patients (5 women and 5 men) with corneal opacities who had the three-dimensional system (Ngenuity, Alcon®) assisted phacoemulsification to remove cataract. In this case series, phacoemulsification with IOL implantation was achieved without intraoperative or postoperative complications. The 3D system was especially helpful for phacoemulsification with corneal opacity and served as a visualization tool, having reduced illumination to less scattering and reflection, enhancing the red reflex, improving depth of field, and allowing precise focusing. So, it can be considered as a useful adjuvant to cataract surgery for providing better visualization in different steps of phacoemulsification with corneal opacity.
